Information on Disclosure, Notice of Purpose of Use, Complaints, and Consultation on Personal Information and Specific Personal Information
All requests for disclosure (disclosure, correction, discontinuation of use, discontinuation of provision to third parties, etc.) of personal information and specific personal information (retained personal data) held by Higashiyama Film Co., Ltd. (hereinafter referred to as “We”, “Our” or “Us”) as well as notifications of the purpose of use, complaints, and consultations will be accepted by sending the prescribed documents by the principal or his/her representative. Please refrain from calling, sending e-mails or visiting as they are not accepted.
1．Documents to be prepared
１） Prescribed request form
Please print the prescribed request form "Request for Use/Disclosure of Retained Personal Data" from the website or call the following contact point. To be able to respond accurately and promptly, we would like to ask you to provide as much detailed as possible about your requests.
Request for Notice of Purpose of Use and Disclosure of Retained Personal Data (PDF:205KB)2） Identification documents
Please submit either of the following two items (1) to (8) as documents for identity verification. In addition, please ensure that one of these points is specified with the current address of the person.
- （1）Copy of driver's license (if the registered domicile is different from the address, a copy with the registered domicile part withheld)
- （2）Copy of passport
- （3）Copy of health insurance card
- （4）Residence certificate
- （5）Seal registration certificate
- （6）Copy of pension book
- （7）Copy of Basic Resident Registration Card
- （8）Copy of alien registration certificate
3) Additional documents for the confirmation of representative (if the request is made by the representative)
If the person making the request is a legal representative of a minor or an adult ward, or an agent entrusted by the principal, please submit a document equivalent to the above 2) and any of the following documents (a) to (d) to confirm the representative.
- （a） If the representative is an attorney-at-law: An attorney number
- （b）In the case of a legal representative of a minor: A certified copy of the family register
- （c）In the case of an agent of an adult ward: A copy of the family register and a copy of the hearing document
- （d） In the case of a voluntary agent: A power of attorney with the principal's registered seal and a seal registration certificate of the principal
4) Return envelope
Envelopes with 420-yen stamps (including postage for simplified registered mail) affixed with the address of the requester (however, this is not necessary in the following cases)
Correction, addition or deletion of retained personal data, suspension of use, deletion of retained personal data, or suspension of provision to a third party
2． Sending of documents
Please send the prescribed request forms, identity verification documents, and return envelopes to the following by simplified registered mail. The documents submitted will be carefully managed as personal information. Please note that we will not return the documents you sent us.

3．Responding to the details of your request
We will promptly respond to your requests within a reasonable period after confirming the arrival of the required documents. We will also provide information on the procedures for subsequent proceedings if necessary depending on the content of the request. Please note that we may not be able to comply with your requests for the following cases.
- ・Cases where the life, body, property, or other rights or interests of the person or a third party are likely to be harmed
- ・Cases that may significantly interfere with the proper performance of our business
- ・Cases in which disclosure violates other laws and regulations
- ・Cases where Disclosure Procedures are separately provided for by other laws and regulations
- ・If it is not possible to confirm that you are the principal

Please note in advance the following points.
- ・We may contact you to confirm the details of your inquiry.
- ・The documents you submitted will not be returned. We will manage it as personal information.
